Rights and Responsibilities

As someone who uses HACC services, you have the following rights which are listed in the HACC Statement of Rights and Responsibilities:

The statement clearly outlines the rights and responsibilities of those receiving HACC services and is part of the National Strategy to promote consumer rights.

The Right to dignity and privacy

Your dignity and privacy must be respected
by service providers, just as you should respect theirs. You should be treated as an individual, taking into account such things as your cultural background or any disability
you may have.


The Right to be assessed for access to services without discrimination

Your access to services should be decided fairly and on the basis of your assessed need and the ability of services to meet that need.

The Right to be informed

You have a right to know which HACC services are available, so that you can make your own decisions. Each service provider can give you information about the service they provide, any costs that may be involved and your rights and responsibilities. They can also help you get information about other services you might need.

The Right to refuse a Service

You are guaranteed that whoever refuses or has been refused the services of Crossroads Gold Coast, will not be disadvantaged from accessing these services in the future.

The Right to choose from alternatives

As well as making choices about the service you want, you have the right to refuse services. Of course, you are responsible for your choices.

The Right to access your health records

You have the right to access your health records upon written request by you or your nominated representative. Information will be supplied within three working days from receipt of your request.

The Right to complain

If you have any concerns about the services you receive, tell your service provider. Complaints shall be dealt with fairly and quickly. A complaint can’t be held against you in the future. Don’t forget, you can use an advocate if you wish. If your complaint
isn’t dealt with fairly and quickly
contact your Commonwealth Department of Health & Aging on (07) 3360 2724.

The Right to an advocate of your choice

If you want you can also involve an advocate (someone who acts on your behalf),
(07) 36376000.

Your Responsibilities

As a participant in the Crossroads
Gold Coast Program, you have general responsibility for the outcome of your decisions and a particular responsibility to respect service providers, staff, volunteers and other consumers.
For Crossroads Gold Coast to work effectively you need to know your rights and responsibilities and be able to assert them. This information will help you obtain the services you need.

Crossroads Gold Coast has developed a number of activities for the Friday program,
and we are always creating a happy and safe environment for all. Crossroad operates under
the auspices of Newlife Uniting Church, Robina and has done so since 1994.

Who can access these programs?

Crossroads Gold Coast aims to provide a quality centre-based day respite service to
families, Carers or other Service Providers who are caring for a person with a disability.

Confidentiality

Because we have to comply with Government requirements, we will need information from
you that will remain strictly confidential and will not be shared with any person or agency
outside of HACC, without your permission or that of your advocate. You have the right to
access your own personal information and files.
